RESUMO

OBJECTIVE: To analyze the spatiotemporal distribution of homicide mortality and association with social determinants of health in the Northeast Region of Brazil. METHODS: Ecological study with spatiotemporal modeling of homicide deaths between 2000 and 2019. Temporal trends were analyzed by segmented linear regression. Crude mortality was calculated and adjusted by smoothing the local empirical Bayesian method and analyzed by the Global/Local Moran Index and spatiotemporal scan statistics. The association between social determinants of health and homicide mortality was performed using multiple linear regression and autoregressive spatial models. RESULTS: 353,089 deaths were recorded. Mortality increased from 2000 to 2019, with an annual increase of 4.37 in males and 3.57 in females. High risk spatial and spatiotemporal clusters were identified in the coastal region of the states. The spatial regression model showed an association with socioeconomic inequalities. CONCLUSIONS: High risk areas for homicides associated with socioeconomic inequality, which should be considered as a priority for designing and investing in public health policies were investigated.

RESUMEN Objetivo: Analizar distribución espaciotemporal de mortalidad por homicidios y relación con determinantes sociales de salud en el Noreste brasileño. Métodos: Estudio ecológico con modelado espaciotemporal de óbitos por homicidios entre 2000 y 2019. Las tendencias temporales fueron analizadas por regresión lineal segmentada. La mortalidad brutal fue calculada y acordada por moderación del método bayesiano empírico local y analizadas por Índice de Moran Global/Local y estadística de barrido espaciotemporal. La relación entre determinantes sociales de salud y mortalidad por homicidios fue realizada mediante la regresión lineal múltiple y modelos espaciales autorregresivos. Resultados: Fueron registrados 353.089 óbitos. La mortalidad aumentó de 2000 para 2019, con un incremento anual de 4,37 en el sexo masculino y 3,57 y en el femenino. Aglomeraciones espaciales y espaciotemporales de alto riesgo fueron identificadas en la región costera de los estados. El modelo de regresión espacial demostró relación con las desigualdades socioeconómicas. Conclusiones: Identificamos áreas de alto riesgo para homicídios relacionadas con la desigualdad socioeconómica, debiendo ser consideradas como prioridad para el delineamiento e inversión en políticas públicas en salud.

Resultados: Foram registrados 353.089 óbitos. A mortalidade aumentou de 2000 para 2019, com um acréscimo anual de 4,37 no sexo masculino e 3,57 no feminino. Aglomerações espaciais e espaçotemporais de alto risco foram identificadas na região costeira dos estados. O modelo de regressão espacial demonstrou associação com as desigualdades socioeconômicas. Conclusões: Identificamos áreas de alto risco para homicídios associadas com a desigualdade socioeconômica, devendo ser consideradas como prioridade para o delineamento e investimento em políticas públicas em saúde.

Introdução:a violência doméstica é um fenômeno que acomete milhões de crianças e adolescentes em todo o mundo, desencadeando diversas consequências a curto, médio e longo prazo. Para enfrentá-la, a Atenção Primária em Saúde emerge como uma importante estratégia vistosuasingularidadenaprestaçãodosserviços.Objetivo:sintetizarasaçõesde enfrentamento da violência doméstica contra crianças e adolescentes na Atenção Primária em Saúde. Métodos:trata-se de uma revisão integrativa com estudos obtidos nas bases de dados BVS MS, SciELO, PubMed e EMBASE, norteada pela pergunta de pesquisa: "Quais as ações de enfrentamento da violência doméstica contra crianças e adolescentes na Atenção Primária em Saúde?". Os estudos investigados abordam a implementação de ações preventivas, através de programas educativos: SOS ajuda para os pais, Safe Environment for Every Kids, Practicing Safety e Play Nicely, e de educação em saúde, além de relatos de profissionais da saúde frente ao enfrentamento dos casos de violência doméstica contra crianças, adolescentes e suas dificuldades. Resultados:a síntese foi capaz de evidenciar diferentes ações de enfrentamento da violência doméstica contra crianças e adolescentes no contexto da Atenção Primária à Saúde, que demonstram sua relevância através de resultados satisfatórios. Conclusão:por fim, fica evidente a necessidade de maiores esforços para seu combate, tanto por meio da disseminação de informações sobre os cuidados às crianças e adolescentes e da capacitação de profissionais de saúde, quanto através do fortalecimento de outros setores da saúde

The coronavirus disease 2019 (COVID-19) outbreak has created unprecedent challenges for healthcare systems worldwide. Oncology services have been reorganized to decrease the risk of nosocomial acquisition of SARS-CoV-2, but changes in treatment pathways and follow-up cancer care can result in patients receiving suboptimal or delayed care. Herein, we describe a cross-sectional nested cohort study conducted to evaluate delays in care for patients with head and neck cancer (HNC) in post-treatment follow-up or palliative care during the COVID-19 pandemic in Northeast Brazil and its impact on health outcomes. Information was extracted from medical records and supplemented by telephone interviews. We compared the following health outcomes: self-perception of anxiety or sadness, fear of COVID-19 infection, cancer-related complications during social isolation, self-medication, diagnosis of COVID-19, and death between patients with and without delayed cancer care. The Mann-Whitney U test was used to compare distributions of continuous variables and the Fisher exact test was used for categorical variables. Thirty-one HNC patients were included in the study, and no case of confirmed SARS-CoV-2 was found. Delayed cancer care due to restriction in health services was reported in 58.1% of cases, and there was no report of telemedicine use during the COVID-19 outbreak. Cancer-related complications during the COVID-19 pandemic were described for most patients (67.7%) and included pain or discomfort, swelling, and dyspnea. Eight (25.8%) patients reported use of prescribed morphine or codeine to manage pain and six (19.4%) patients reported self-medication with over-the-counter (OTC) non-steroidal anti-inflammatory drugs (NSAIDs). We found an association between delayed HNC care and the use of self-medication (p = 0.028). This study indicated that patients with delayed HNC care during the COVID-19 outbreak are more likely to use self-medication with NSAIDs for pain management. Better strategies to follow HNC patients in socioeconomically disadvantaged communities need to be discussed and implemented.
